چکیده

The objective of this study was to investigate the risk factors for leukoaraiosis or white matter hyperintensities (WMHs) in patients with cerebral infarction and the relevance of lipoprotein-associated phospholipase A2 (Lp-PLA2) levels to leukoaraiosis. Two hundred and sixty seven patients with cerebral infarction were categorized into two groups of patients with leukoaraiosis (121 cases) and those with no leukoaraiosis (146 cases). The clinical and laboratory variables including Lp-PLA2 levels were compared in both the groups. The morbidity associated with leukoaraiosis was 45.3% in the 267 cases of cerebral infarction. One-way analysis revealed significant differences between the leukoaraiosis and non-leukoaraiosis patient groups in terms of age, Lp-PLA2 level, and history of hypertension and lacunar infarction (LACI). Multivariate logistic regression analyses were performed to compare the contribution of Lp-PLA2, age, and LACI between the two groups. We did not observe a statistically significant difference between the leukoaraiosis group and the non-leukoaraiosis group in terms of sex, diabetes mellitus history, hyperlipemia, smoking, drinking, coronary disease, and cerebrovascular stenosis rate. The levels of low-density lipoprotein (LDL), triglyceride (TG), total cholesterol (TC), and fasting blood sugar had no significant differences between the two groups of hyperlipoproteinemia (HLP) patients. Further analysis of cerebrovascular stenosis categorized into simple anterior circulation, simple posterior circulation, or multicirculation infarctions, did not show any statistically significant differences. Age, Lp-PLA2, and LACI were found to be independent risk factors for leukoaraiosis in cerebral infarction patients. Hypertension was the significant risk factor for leukoaraiosis whereas stenosis in the extracranial and intracranial arteries had no definite relationship with leukoaraiosis. Furthermore, diabetes mellitus, hyperlipidemia, smoking and drinking history, and CHD were the common risk factors for leukoaraiosis and stroke.

The Frequency of Cerebral Microembolism in Acute Myocardial Infarction

ABSTRACT Introduction: Stroke is more common in patients with cerebral microembolisms. Frequency of cerebral microembolisms (high intensity transient signals, HITS) in acute myocardial infarction has been reported about 17%. The factors that influence on microembolism after myocardial infarction (MI) are not definitive. Type of MI, Ejection fraction, Hx of Streptokinase is the factors that were...

Risk Factors for Leukoaraiosis in North American and Iranian Stroke Patients

Background: Leukoaraiosis (LA) or white matter thinning and rarefaction are common in stroke patients. This pilot double-center study was designed to evaluate LA risk factors in stroke patients. Methods: This cross sectional study was conducted on 100 consecutive stroke patients in Walter Mackenzie Hospital, Canada and 100 consecutive stroke patients in Valie-Asr Hospital, Iran in 2004. Diagnos...

The predictors of subclinical cerebral infarcts in ischaemic stroke patients.

Subclinical cerebral infarcts (SCI) have been increasingly shown to cause a significant clinical impact. However, there are limited data available on Asian patients. The objectives of this study were to determine the prevalence of SCI in ischaemic stroke patients seen in the Hospital Universiti Kebangsaan Malaysia (HUKM) and to identify its associated risk factors. We evaluated the brain comput...

The Presence of Previous Cerebral Microbleeds Has a Negative Effect on Hypertensive Intracerebral Hemorrhage Recovery

Background and Purpose: Cerebral microbleeds are an intracerebral microangiopathy with bleeding tendency found in intracerebral hemorrhage patients. However, studies about cerebral microbleed effects on the prognosis of hypertensive intracerebral hemorrhage patients are rare. We performed a prospective study to discuss not only the risk factors of cerebral microbleed incidence in hypertensive i...

Leukoaraiosis and Increased Cerebral Susceptibility to Ischemia: Lack of Confounding by Carotid Disease

BACKGROUND Leukoaraiosis is associated with an increased risk of stroke, but the underlying mechanism remains uncertain, as do the associations with other risk factors, such as carotid disease. We aimed to determine the role of carotid disease and of other clinical variables in the development of leukoaraiosis and to define their contributions to the associated increased risk of stroke. METHO...
